Bridges to Health
Bridges to Health is located inside the St. Martin Community Center at 901 S. Branson St., Marion, IN 46952. To schedule an appointment or get more information, call 662-7289 during their hours of operation which are Tuesday and Thursday evenings between 5:00 pm and 8:00 pm. All visits to the clinic are by appointment only.
Patient Guidelines
- Uninsured residents of Grant County
- Age 18 or older
- Limited services for Medicare and Medicaid recipients
- Must meet financial guidelines
Services Provided

Primary Care (ear infection and strep throat)

Chronice care management (high blood pressure, diabetes)

Annual physicals

Medication assistance (no narcotics on site)

Referral source to additional services
Volunteer & Financial Support
Services are provided by volunteer healthcare professionals and social workers. Community volunteers are considered "partners in care" and are critical to our mission. They greet patients, guide them through the healthcare process, and offer compassion and concern. Bridges to Health community volunteers provide clinical, operational and administrative support. There is an ongoing need for strong volunteer partnerships. Please contact Linda at 765-662-7289 about volunteering at Bridges to Health.
